Cara Care: from a call for help to a referral.
Cara Care helps young people aged 15 to 23 find their way back into society, with small-scale supported living, crisis accommodation and its day programme Cara Boost. DTB WEB built the website where referrers, parents and young people find the care on offer and refer a young person. The emergency number sits at the top of every page.

The care on offer
Six forms of care each have their own page: supported living with intensive guidance, crisis accommodation, outreach support, treatment, daytime activities and Cara Boost. Each one says briefly who it is for and where it takes place or how long it lasts. An overview page sets them side by side, with the municipalities Cara Care is contracted by through GRJR, Mevis and SOJ Zuid-Holland Zuid.
A route for every visitor
Referrers, parents and young people each look for something different. On the homepage you choose I am a referrer, I am a parent or I am a young person, and read what matters to you: short lines of contact, working with parents or what living there is like. A timeline shows how a young person moves on: from crisis accommodation through smaller homes and a studio of their own to a front door of their own.
Referral in four steps
The referral form asks in four steps about the young person, the referrer, the request for help and the funding, such as the Dutch Youth Act or the Wmo. A copy of the care decision can be attached. Those who prefer to call or email find the referral team and the emergency number beside the form. On the homepage, three steps explain what happens after a referral.
The delivered website
Besides the care on offer, there are pages about the organisation and the team, stories from practice, working at Cara Care and contact. The rights of young people have a place of their own too, with the complaints procedure and the confidential adviser. The website has a dashboard of its own for new stories and pages. The website is live at caracare.nl.
